Creating New Bsns Contact Form

When trying to create a new bsns contact, I can no longer
get the nice form that had categories (masterlist) as
well as some categories that I had added to it.  It seems
that since I downloaded microsoft office 'updates' w/i
the last week-10 days, I cannot get a screen to add
a 'new bsns contact' at all. The message I get is: "The
form required to view this message cannot be displayed.  
Contact your administrator."

Clearing the cache usually fixes it. Choose Tools | Options | Other |
Advanced Options | Custom Forms, then click Manage Forms, then Clear Cache.
