Can't access address book from Word

Hi,

My Word 2000 does not let me access my Outlook address
book anymore when I want to print out envelopes and
labels. More specifically, when I am in Word
and want to print envelopes or labels I click on the
address book icon and a  "Choose Profile" box pops
up.

The "Profile Name" field is now showing "Microsoft Outlook
Internet Settings".  I don't have any idea how to change
this, or what it said before. My daughter originally set
this up for me and I think she typed in a profile name,
then followed the instructions to set up a path in order
to use my Outlook address book.  I have no idea how to
reset this or what the path SHOULD be.  Can anyone help?
This was working fine last week -- I don't know what
happened.

System info is:
Windows ME
Outlook 98
Word 2000
