Contact subfolders not available when in mail merge

I am walking a user through the mail merge process. User
has many contact sub-folders.  User does not have access
to contact sub-folders when using mail merge in Word.  

Any suggestions on what may be causing this. I recreated
user's profile in Outlook and all folders are avialable
from his mailbox.  Outlook is opened when in Word.

Did you enable the subfolders as email address books? R click > Properties >
Outlook Address Book Tab.
