merging files into a custom form

I have created a custom form to import sales leads into a separate folder in
outlook. When the import wizard gets to where I have to "map custom fields"
my custom fields are not there. When I go to "show fields" in the folder my
custom fields are there and the custom form comes up when I go to enter a
single contact. I entered all of my custom fields into the "field chooser"
under "user defined fields" as I was designing the form. How do I get the
custom fields into the "map custom fields" box.

Outlook doesn't support importing to a custom form. You'd have to write
custom code or use a third-party application. See
