Sporadically when I enter a new contact the name gets
reversed in my address book.  For example, I enter Mr.
Wally Smith and in "file as" window it shows Wally as the
last name and Smith as the first name.  It alphabetically
stores it as Wally as the last name even through in the
contact name fields they are in the proper place.  It
doesn't happen all of the time and I can't figure out a
consistent error about it.

What default setting did you specify for the File As field?
