How do I import data into a custom designed form in contacts?

Hi. I have designed a custom form in Outlook to manage my contacts. I have
created a new sub-folder within Contacts and set the custom form to be linked
to this folder. When I manually add a new contact the correct form comes up
but when I import data(from database to do telephone canvassing) into this
subfolder the standard form, (not the one I have created)is what appears for
these new entries. How to I get the imported data to import to my customised
form?

The functionality you're looking for isn't supported via Outlook's import
process. You'll need to write your own code or use a 3rd party product (such
as ContactGenie - info link in sig line below). You can also find other
alternatives on the Slipstick site
