Until I upgraded to Outlook 2003, I was able to include the sub-headings in
my Contacts entries (eg Home Address/Business Address etc) when printing an
Address Book (Medium Booklet Style, should that be relevant). Now,
subheadings for Full Name, phone number and e-mail categories are shown, but
NOT those to distinguish (say) Home address from Business (or Other)
addresses. The result is a very muddled address book.

Any suggestions for a cure?

You did not include enough information for anyone to know how you were
printing your address book before, so none of this makes any sense.
Most of us use mail merges to print out list of our Contacts because they
provide the most features and flexibility to construct any sort of printout
you want.
