How can I customize & add fields to my Contact folder

I have a Media list I want to import into Outlook.  There
are 3 fields that would be customized fields.

How do I create a view containing these 3 fields and label
them clearly not just as User Field 1, etc.

Outlook 2000, Windows 2000
approx. 3000 contacts.

Outlook doesn't support importing to a custom form. You'd have to write
custom code or use a third-party application. See
http://www.outlookcode.com/d/customimport.htm.
