There should be a way to map customized contact forms in order to.

I have created a customized contact form to use however I cannot map the
customized form (only the form that is generic and comes with contact). If I
cannot map this form ALL the information on the excel spreadsheet has to be
manually input with the except of a few fields.

I cannot find any other information on the web on how to accomplish this.

Outlook doesn't support importing to a custom form. You'd have to write
custom code or use a third-party application. See

If I understand your problem, my first suggestion would be to export your
Contacts into an Excel spreadsheet to see where/how Outlook is labeling the
field(s).  Match that with the contact info you're trying to enter, then
Import.

No. Outlook doesn't export or import custom fields. See
