New MS Word document to Contact from Outlook ?

   I would like to:

   Create a new Word document from a Contact in Outlook.

   I would like to be able to use a previously selected template

   I want the name and address information to be entered automatically in
the document

   It would be nice if it provided a default name, not conflicting with any
existing name using part of the contact's name

   I want it to create a journal entry for the document in the contact's
journal.

   How close to this can I come?
