sending contacts a query or template

I would like to send a query, template, and or a form via e-mail to all my
contacts requesting of them to update (for my records) their personal
information, that they could fill out and return to me.  Frist what kind of
form might i use or where might i find it? Second, is there a way to do it to
where when they reply their information automacically plugs into my address
book with me having to do each one separtely.

Since there's no way to know what mail program a person might be using, the
only form that might work would be a web-based form. There are online
services to do this -- see http://www.slipstick.com/addins/contacts.htm .
Some us find them intrusive and delete all such requests.
