Can a field in a new form be formatted as a phone number?

I am trying to building a new form and format a field so it will
automatically format as a telephone number.

You would have to write code to do this. The easiest way is to create
another dummy contact, put your text into one of the other contact's phone
fields, and let that item "resolve" the number for you. See
