When using address book to enter an address in Word, some
addresses are omitted.  Only name and country is
inserted.  Additionally I noticed that import/export in
address book is disabled.

You aren't telling us enough to for anyone to help. At a minimum we would
need to know:
Outlook and Word versions, including mail support mode of Outlook if earlier
than version 2002.
Configuration of the Outlook Address Book
Precise steps you are using to insert the Address, including the fields you
are choosing.

Import/Export is not an address book function. It is done from the Outlook
file menu.
