I'm trying to import an Excel spreadsheet to an Outlook
2002 contacts folder. When I get to the "map custom
fields" dialog box, the user fields I added to the
contacts current view don't appear, and there is no
button for adding new fields in "map custom fields" (as
Excel help says there should be).

The Outlook import/export function doesn't support user-defined fields.
You'll need a 3rd party product to accomplish that (like ours - link below).
You can also look at http://www.slipstick.com/addins/contacts.htm
