Outlook inserts SMTP: in front of email address, and then refuses to send
emails with SMTP: in front of them

Your msg is a tad cryptic to be able to provide any kind of detailed answer.

1) Outlook version?
2) How was the CSV file created?
3) Assume the CSV file contains email addresses with SMTP at the start. Is
that correct? If it is - then your issue is in the data contained in the CSV
file which would need to be fixed in step#2 when the file is generated and
before being imported into Outlook.
