Customize list of names and e-mail addresses displayed

I want to change the way names are listed when I pick the "to" button while
writing an e-mail. I recently upgraded to Outlook 2002 SP3 and now the
listing shows the name twice (first column as "name" and second as "display
name".  Can I eliminate one of them since they are identical?

Also, it shows some people's names twice (or more).  For instance, it list
them first with an e-mail, then it repeats on the next row with a fax number.
Is there a way to change that to only show them once (for e-mail only)?

No. FWIW, the display name can be customized to have just the email address
or something like "John Smith - Home" so you can more easily identify the
address.
