In My Contacts there are several folders which are now empty.  How can I
permanently remove them?

Right click and Delete. If Delete is greyed out, you cannot.

What to do if DELETE is grayed out?

You'd need to explain how that happened. Delete is only grayed out for the
default Contacts Folder, and there can only be one Contacts Folder.

Under My Contacts, there is: Contacts, Address Book, + 3 other named contact
folders. Yes there is still another Contacts Folder which is empty.  How it
got there I do not know.

Use the Folder List view to give us a more accurate depiction of what
Contact Folders really exist in your profile.

Under Folder view we have the following:
Personal Folders
 Calendar
 Contacts
    Address Book
    Apple
    Avalon
    Contacts
    NYC-DOE
    Victory

(All of the above are populated with different contact entries)

Additional question:
In Folder view after Personal Folders, then Archive Folders, there is
another Personal Folders which replicates the top level personal folders.
Why?

In Contacts View the list is as follows:

MY CONTACTS
 Address Book
 Contacts
 Avalon
 NYC-DOE
 Victory
 Contacts (Empty)
 Address Book in Persoanl Folders
 Contacts in Personal Folders
 Apple in Personal Folders
 NYC-DOE inPersonal Folders
 Victory in Personal Folders
 Contacts in Personal Folders (Empty)

Well no wonder. What is that second set of Personal Folders doing there? How
did you manage that?
There is more to this story that you're telling us.

Well, I do not know how I did that.  Can I safely remove the 2nd set of
personal folders as a start?

Hopefully. It depends on how it got there. If your profile isn't corrupt, you
can just R click and close this second set. See if it contains any
information you want before you do, and make sure you note its name and
location in case you need it back.

Well, I removed the "Victory" folder from the 2nd set of  Contacts in the 2nd
set of personal folders and it also took it out of the primary set of
personal folders.

I did not need that set of contacts anylonger.

What to do next.  Totally confused.
