I have created a new form using Outlook 2000 running on an MS Exchange
server 2003. I also have access to Outlook 2003 if that is of any
assistance. I am trying to export the fields to a database so I am able
to track the information that is passed back to me. Does anyone know of
what I need to do to create fields that can be exported out of Outlook?

Outlook doesn't support exporting custom fields. You'd have to write custom code or use a third-party application. See http://www.outlookcode.com/d/customimport.htm .
