Outlook not connecting to <Server> (it asks for password, there isn't one,
user is authenticated at logon). You end up having to go to lower right
corner of the Outlook window where it says "Offline" and tell it to connect
(or close Outlook and kill a lingering process). Outlook is set to
automatically detect the connection state.
Could it be a server issue?

firewall on either the server or the client side (special attention on the
RPC ports)
also check for blank passwords... some features don't work well with blank
passwords.
and most obvious... can you ping the Exchange server from the client
machine? Is the AD server running correctly? are you logging on to the
domain or the local machine?
