Lookout disappeared from Outlook

Outlook crashed and gave me some error stating that Lookout was
affected.  Unfortunately, I didn't pay much attention to the error message at
that time.  The next time I started Outlook, it was no longer showing up
regardless of whether or not the Lookout Toolbar was selected to be visible.
I have since uninstalled Lookout, restarted my computer, and resinstalled
Lookout.  Outlook does not  recognize that Lookout has been installed and it
does not list the Lookout toolbar as an option.  Any suggestions?

Most likely, LookOut (as an Outlook add-in) has been disabled when
Outlook crashed.

Outlook added it to a list of add-ins not to run.

To restore it, go to:
Help -> About Outlook -> Disabled Items (button)
Select the disabled add-in and remove it from the list.

(may need to restart Outlook afterwards).

I believe LookOut has FAQ and more information about this on their
website.  But this is a common problem for all add-ins.
