Need you advice regarding custom distribution list form

Hi,

I need to develop a custom distribution list form,which display more columns
in the first page
than the originial form
I noticed that i cannot remove the first page in a distribution list form so
i wonder
can i use a custom contact form for a custom distribution list?

i just need to display agrid in the first page with the following columns

rmail
name
field 1(come from my data base)
field 2(come from my data base)

btw,instead of stroing  field1,field2 in my data base,can i some how attach
it to a contact with out getting security warning
about script etc?

If you use a custom contact form, the item stored will be a contact, not a
distribution list.
