Custom form

Hello all,

I have a custom form which has a number of user fields in. i would like
Outlook to push the data to an Access database on the Click of a
button.

I have been struggling with this for a little while and was wondering
if someone could start me of with and example of the code needed,
perhaps with Field1, Field2 examples etc etc

I would really appreicate this
