Customized Form Problem

I designed a customized contact form several years ago.  It was working
great, but we needed to add an additional field which is a yes/no check box.
I updated/published the changes and all the desktops can access it just
fine, except one.  One computer locks up when opening any contact using the
form and the form does not appear the same as the others.  Some fields are
not appearing at all.   Any ideas?
(We have Outlook 2000 and Exchange 5.5.)

Where is the form published? Did you increment the version number? Try
clearing the forms cache on the problem machine.
