data base connectivity

hello sir,
i customized the msg form with 2 fields name and age.i created an access
mdb as my backend.
what i need is when my form opens the the text box must fill with my first
data set (name,age).

for this how can i write the coding in item_open() event?
i created a user DSN for my database  in access.is it need to cereate a file
DSN?
