Update:
I can get it to work by adding [From] to the subject field, but only if I
use Outlook 2000 or greater to create the form, and only if I read it from
Outlook 2003 with the "reading pane" enabled.
Anyone know how I can get it to work older versions of Outlook and not have
to have the reading pane enabled?

I also had to add the From field to the form as well.
