Created a Form in office 2k - details disappearing

Can anyone help ive created a form in outlook 2000 and then published i
but whenever i enter details on the second page and then send it, th
recipient recieves this ok but all the fields on the second page ar
blank.

Is there an option that i have to change so that the values on the 2n
page remain ??

Im sure this must be something really simple but i cant find th
answer. Any ideas welcome.

You have to send the item using the TNEF wrapper.  Open the CONTACT item
for the recipient, select the email address, right-click it, choose
properties, and, on the bottom is a check box labeled "Always send to this
recipient in rtf".  Check it.  Then save the contact record.  This will
ensure that the TNEF wrapper is always used when sending an item to this
recipient, even if you want to use HTML, or the Word editor.

Another possibility is that you made the form with separate compose and
read displays, and didn't fill anything in the read display.  Seems
unlikely, because you say the recipient sees the fields, just not the
data.  Still, for some strange reason, split forms is the default.
