Three different ways, none of them show my form:
1.) I look in tools, options, other, advanced options,
custom forms, manage forms.
2.) Under the properties of an object, then choosing the
forms tab.
3.) In Exchange System Manager, under system folders in
public folders. No forms show in the forms registry.

Is this Outlook 2003? Are you running in cached Exchange mode offline
without synching forms?
I can't think of any other reason why forms would suddenly vanish. Of
course, those in your Personal Forms library are specific to your Exchange
mailbox. If you changed mailboxes or default data stores, you wouldn't see
the same forms. FYI, you can't view forms with the Exchange System Manager
at all.
