I've heard reports of some other problems related to custom icons in Outlook
2003, but don't have any steps to reproduce any of them. Details on how you
created your icons might be useful, or post them where other people can
download and test the issue.
Is this a public folder? If so, are you accessing it through the
PF\Favorites hierarchy? That's inadvisable, unless you've installed the
hotfix for OL2003 PF\Favorites form problems.

Thanks for your quick response.
I think I found the problem. It was because I saved the
icons with true colors. I tried 256 colors and got the
same result, but when I saved them with 16 colors, form
started functioning correctly. They don't look as nice,
but that's ok.
This form works fine with icons saved with true colors in
Outlook2002. The error occurs only on 2003. Very Bizzare.
By the way, the error never actually showed up when custom
action was triggered programmatically using
Item_CustomAction.
