Where can I see examples of forms?

I am obviously new to forms with outlook.

I am looking to make a special order form for a retail store.  This form
would need to have about 15 - 20 fields on it.  It is my plan that it would
get sent back & forth to store & office with information updated at both
locations.

1. is this possible
2. Does anyone have an example that they can e-mail me or a website I can
see one to get examples???
3. Would anyone want to make it for a fee?
