Form script hangs on form open

I'm running OL2003 on a windows network. We use a custom form from an
organizational forms library for travel requests. The first thing this form
does on opening is to pre-load the user's email address, plus the email
addresses of the management chain, into a routing address field. In normal
operation, the OL security popup (a process is trying to access your address
book. Click here to allow acccess) would appear when loading this form.
On my computer, the popup never appears, but the form hangs until:
a) I terminate the app (which closes Outlook)
b) I press the right key sequence to cause the form to cancel, with a
message that states "you must select "yes" in the security dialog in order
to process this form"
c) I press (by sheer luck) the right key sequence to get the form open.

It appears that the dialog code for the security popup is running - but hte
dialog box never show on-screen.

Any ideas?
