Custom Contact Form

We'd like to change the default fields on the Contact Form
and have the new form be the company-wide default.  
Creating the new form is easy enough, the question is how
do we make this form the default for everyone in the
company?

Everyone uses personal folders here and the box at the
bottom of the Forms tab isn't enabled.  The help screen
says that these properties don't work for personal
folders.  Is there any way around this other than making
everyone use public folders?  (We really don't want to
have to maintain them.)

Publish the form to the Organizational Forms library, then make the necessary registry change to make it the global default. See http://www.slipstick.com/dev/newdefaultform.htm#sub
