Creating a form

Hi.

I work in a hospital and we have an employee recognition
program.  I want to know if I can develop a form for
employees to use via Outlook to submit a nomination to me
electronically rather than having to find a form, filling
it out, and bringing it to me.  Does anyone have a
suggestion where I might find information on how to do
this?

Yes, if you're all using Outlook, you can do this with a custom form. See
