MS Outlook Publish Request Form

Hello,

I am trying to publish a form created in Word to MS Outlook.  This
form has three drop down menu's (each having two-three options), eight
text boxes (two of which are dates).  And a number of check boxes.

I cannot seem to get this form published to Outlook correctly.  When
people call up the form, they are able to edit the fields (i.e.
move/delete them).  I have also tried creating the form in Outlook but
am experiencing problems doing this.

The link at the bottom shows the Word form and what I am trying to do.

A form created in Word is not the same as an Outlook form and can't be
published as an Outlook form.
