how do I delete a custom form in Outlook 2002?

I have set-up a couple of custom forms in Outlook 2002 and would like to
delete one but in the Choose Form window, the delete button is greyed out.
I did a search in Windows Explorer which came up with one of the forms as a
folder but not the other (which has spaces in the filename).
