Appointment form problem

I have a couple of questions that I hope someone could solve:

Q1: I have to create a custom form for the calendar.  Can I create a
custom form that pops up when I double click a time slot?  If so how
do I do it? Also can I change the default first page of the
appointment instead of adding another page?

Q2: The other question I have is if I can create the form above can I
show different fields in the default view.  (Where it show the time
slot can I have it show something other then Subject (Location)?

1) Publish your custom form and make it the default for the folder. See

2) In a table view, yes. In a day/week/month view, no.
