Outlook 2003 View Control

I'm using the standard Vacation request form sample provided by Sue Mosher I
believe.  Have had everything setup & working for some time now with no
problems.  Normally when the manager receives a vacation request form, the
vacations tab within the form shows the public folder vacation calendar.
This still works fine when opening the from with previous versions of
Outlook, but not with Outlook 2003.  I just get a blank page, no error
messages or anything.

Also, when the manager approves the vacation request, the original request
is added as an attachment for the end user to drag onto their calendar.
This does not happen anymore when the approve button is clicked.

Does anyone have any input here, or has experienced the same thing.

Is the manager working with items in his/her own mailbox or in a secondary
mailbox? Where is the form published?
