Where can I learn more about creating Outlook forms

I am using Outlook (Office XP). I remember that the Help files used to
describe how to create Outlook FORMS and how to set up a Personal Outlook
Forms Folder.

Now that I am interested in doing that, there doesn't seem to be much
information.

Can someone suggest a URL that can get me started?
