I have design contact and task form changing both the large and small icons,
replacing them with a another icon file. However, when ever I use the form
the standard Outlook icon is used. what I am doing wrong? I have checked that
the "send form definition" is not checked. I am running XP and Outlook 2003.
Any help would be much appreciated.

Check the MessageClass property of saved items to see whether something else is causing a one-off. Also, are the icons you used 32x32 and look OK on the (Properties) page?
