Custom task form - No paper clip icon and can't insert file

I am designing a custom task form in Outlook 2003 and am running Windows XP
Pro SP2.

When I run the form, there is no "paper clip" icon and the Insert menu only
has Item (but this is greyed out). I had a feeling that at some stage my form
did allow attachments (I have a text box that counts the number of
attachments, and this used to work), but it doesn't now.

Have I done something really simple and dumb, or will I need a command
button to mimic some of the paper clip functionality?

The most likely cause is that you removed the message body control, the large box on the bottom of the default page. You'll need to add it back by dragging the Message or Notes field from the Field Chooser to your form.
