help with educational forms

hello.  i use Outlook quite heavily as part of my job as a teacher.  I use
the calendar with appointments to hold lesson plans and the journal to
record meetings or phone calls with parents or pupils.  I have tried to
create my own customised forms but not had much look - mainly because of
getting the dates on to the forms correctly.

I would be grateful if anyone has come across or even created useful forms
for educational use if they could let me know where or how to get hold of
them.
