This organizational form no longer displays properly since upgrading
to Office 2003 SP2.  No error messages are generated, the form simply
doesn't display properly.  It displays as a regular e-mail message
with no fields, rather than the way it should look.  The form opens
fine on systems that have not been updated to SP2 yet...

Any ideas??

Thanks in advance for any help....

If you are using an .oft file, rather than a published form, you'll need to run it with Tools | Forms | Choose Form.
