Design Forms: add field for user definable photo

MS Office PRo Outlook; Design Forms:

1- What field do I use to allow a user entering contacts to upload photo of
contact?

2- Also, how do I add a new birthday/anniv date field for user to enter a
date corrresponding to a semi-annual event that the user needs to be alerted
for semi-annually not annually?

1) You use an image control, as described at http://www.outlookcode.com/d/formpicture.htm#control.

2) You'd have to add not just a date field, but code behind the form to create the related appointment.
