Modifying Company Holidays Outlook Form

I've downloaded the custom form from Slipstick for distributing company
holidays and used it in both the Organizational Forms folder and my Personal
Folder successfully.  But as soon as I modify anything in the form (text) and
publish it back to my Personal Folder, the script that copies the dates to
the Outlook calendar does not work.  The script to load the form with the
dates works fine, it's just the script behing cmdAddHolidays that quits
working.

Will it only work if published in the Organizational Forms folder?  Is it
related to "one-off"?

Any help would be appreciated.

I should add that I've made sure not to check the "Send form definition" on
the forms properties and when I publish it.
