Outlook forms design

I have created a form in Outlook but the Message box (frame) is blank when I
send an email. Also I would like to have the form visible on the preview
pane. Can someone please advice?

Have you published the form somewhere?  Have you set the target email address's
format property to always send in rtf?  You do this by opening the contact item
containing the target e-mail address, right-clicking the e-mail address field,
choose properties, and then select the rtf format from the dropdown listbox in
OL2003, or checking the box labeled always send to this address using rtf.
