I am trying to create a questionnaire form using outlook.  I would like to
save all responses in an access db.  What is the best way of doing this.

Who is the audience for this questionnaire? It's quite possible that a custom Outlook form is not the right solution.

The audience is people from my department.  I remember being able to link an
HTML form using FrontPage.  However, that will require ASP.  Is there a way
to just get the results of a form and export it out to access automatically?

Nope. You'd have to write your own export code or use a third-party tool to get the results into Access.

And to make the form work at all, you'd need to be able to publish it to the Organizational Forms library on Exchange.
