viewing custom forms in outlook after sent to another user

Hello.  I created a custom form in outlook with some simple text fields
labeled as to what information should be in them, i saved the form, published
it.  when i send it to others the message simply shows up as a blank email.
im no expert, but i have read thru the help menus and have tried everything
i know.
suggestions?
