How do I print a custom email form as it looks on the screen?

In Outlook 2003 I modified the email form and created a new form call 'VPO
request'.  for my purposes every thing works just fine, sort of like a
website data entry without any data storage. simply a request form.  However
when I print the form after it has been emailed it prints the form entries in
a list format (like the tab order screen) with the inputs after a colon.  
Rather than printing the form like the format on the screen.  I would like it
to print just like the form is laid out on the screen.  Is this possible and
how do I do it?

See http://www.outlookcode.com/d/customprint.htm for various solutions to the limitation that Outlook forms don't support WYSIWYG printing. The Word template technique is the most flexible and highly recommended.
