I created a form that allows the sender to enter some basic information.  I
publish the form to my Personal Forms Library, and then send it to myself to
test.  I receive a message using the standard email form.  I have set up both
the Edit Compose Page and Edit Read Page to be identical, but I still can't
view the form properly.  Any suggestions?  Thanks.
