Hello Sue,
The icon for the IPM.Note custom form with vbscript has been assigned 2 new
icons (large and small) on properties.
I've unchecked the "Send form definition with item"
I've then published it to my personal forms library.
Upon selecting the "run this form" it doesn'd change the icon.
Ideas?

Can you provide more details please on what you're doing and what you hope to accomplish ?IPM.Note is not a custom form. It's the standard form for messages. And running a published custom form isn't going to "change the icon" on any existing items. Furthermore, the way to "run" a published custom form is with the Tools | Forms | Choose Form command. Messages created that way should show your custom icon, assuming it's the right size and color depth, like the standard icons.
