Creating an outlook form for Phone Messages

I would really like to create a custom form to manage phone calls.  Something
with customizable fields.. Say a drop down message for who the message is
for, and an auto Date/Time feild etc.

I'd think that someone has already created one someplace that I could
install onto my system here, instead of having to figure out how to create my
own form.

There should be one listed at http://www.slipstick.com/addins/mssampleapps.htm. Note that it will need to be published to the Organizational Forms library or to each user's Personal Forms library.
